Until further notice, we are implementing the following policies:

  1. Masks are required to be worn by patients and any visitor or service may be denied.
  2. All persons entering our building will be screened for COVID-19.
  3. No visitors may attend appointments with any patients unless the patient is a minor and both gaurdian and minor has been screened. We apologize for the inconvenience, and we will work to reschedule any appointment if you need to find a babysitter.
  4. Please logon to the patient portal and fill out your patient paperwork prior to your appointment. (If you are having trouble logging into your patient portal, call 817-741-9663 during normal business hours, and we’ll get your password reset.)
  5. Please note, our office does not provide COVID-19 testing.
